Cost/Insurance

We accept most major insurances. 

A few very important things:

1. Be sure to check your annual DEDUCTIBLE. You may be charged full price even if we accept your plan.

2. It is always a good idea to call your insurance to see if Pride Health Clinic is in network and the provider your plan to see.

3. Even if we don’t take your insurance, your plan may still cover your labs and/or medicines. You can still send a billing form to your insurance for out-of-network coverage. Please ask if you need this form.

4. Please see the list of insurances below that we currently have contacts with.


Insurances Accepted:

Check each provider to see if they are in network with your insurance before scheduling. If you are seen and they are not in your network, you will be responsible to pay for the visit charges

*VA Benefits can be used if patient contacts VA to start the process for the referral and contract. We will need VA patient's Social Security Number to file claims.

**If your insurance is not listed, please contact us and we will submit to become a provider. This process takes about 30-90 days depending on your insurance company.
